Five years ago this week, Massachusetts became the first state to move toward universal health care for its residents. Now, Massachusetts is considering further reform–this time in the realm of health care payments. Proposals are being considered to replace the fee-for-service system. Governor Deval Patrick has emphasized that cost control is important and is proposing a new type of payment mechanism. His idea is to give lump payments to teams of doctors reponsible for almost all of the care of a group of patients, with incentives given for cost-savings and the provision of high caliber care that keeps patients healthy.
